Esansiyel hipertansiyonun sol ventrikül diastolik fonksiyonu üzerine etkili bir faktör olduğu bir çok çalışmada gösterilmiştir. AMAÇ: Çalışmamızın amacı esansiyel hipertansiyonda sol ventrikül kitlesi ile sol ventrikül doluş özelliği arasındaki ilişkiyi göstermekti. YÖNTEM: Bu amaçla polikliniğimize gelen yaşları 50-80 arasında olan 118 hipertansif hastayı değerlendirmeye aldık. SV kitle indeksini belirlemek için American Society of Echocardiography'nin kriterlerine göre M mode ekokardiyografik ölçümler yapıldı. Pulsed Doppler ekokardiografi ile üç ayrı mitral akım örneği belirlendi: bozulmuş relaksasyon [E/A0.05] en sık olarak belirlendi. Buna ek olarak bozulmuş relaksasyon örneğinin görülme sıklığının yaş ile arttığı görüldü. SONUÇ: Bu bulgular esansiyel hipertansiyonda değişen sol ventrikül diastolik özelliğinin sol ventrikül kitlesinden bağımsız olduğunu ve bu değişikliğin sol ventrikül hipertrofisinden çok miyokardın yapısal özelliğindeki değişiklik ve yaştan kaynaklanabileceğini düşündürmektedir.
The effect of essential hypertension on left ventricular (LV) diastolic function has been noticed in several studies. PURPOSE: The main purpose of our study is to show the relation between LV mass and LV filling characteristic in essential hypertension. METHOD: Ear this purpose, we evaluated 118 prospective hypertensive patients (aged between 50-80) from our outpatient clinic. M mode measurements were obtained according to criteria of American Society of Echocardiography to calculate LV mass index. Three distinct mitral flow patterns were identified with pulsed Doppler echocardiography; impaired relaxation pattern [E/A < 1 and prolonged E deceleration time(EDT) according to age/, restrictive physiology (E/A > 2.1, EDT< 150ms) and normal (E/A>1, EDT>150 ms). Prevalence of mitral flow patterns according to age groups (50-59;60-69;70-80), genders and presence [SVH(+)] or absence [SVH(-)] of LV hypertrophy were compared. RESULTS: A significant difference in the prevalence of mitral flow patterns couldn't be found between patients with and without LV hypertrophy. Impaired relaxation was most frequent mitral flow pattern in both groups [SVH(+)%69 - SVH(-)%64,4 p>0.05]. Additionally, increased prevalence of impaired relaxation pattern with age was shown. CONCLUSION: These findings suggest that altered LV diastolic properties in essential hypertension is independent of LV hypertrophy and, may result from the changes in the structural properties of myocardium and age rather than LV hypertrophy.
