It has been well established that the renal functions of patients with essential hypertension may be essentially normal during the initial stage of the illness (6, 14). As the disease progresses, however, they become gradually impaired. This functional impairment may result from organic changes (arteriolar sclerosis, arteriolar necrosis or superimposed pyelonephritis) and/or from purely hemodynamic factors (arteriolar spasms, congestive heart failure). The degree of functional impairment, the rate of renal deterioration, and the significance of the etiological factors are extremely variable in different patients.
