Study and correlation of the symptomatology and the effect of ultrasound treatment of essential tremor through standardized scales and medical imaging

[ES] El trabajo de fin de grado propuesto se basa en el análisis del tratamiento por MRgFUS de pacientes con temblor esencial. Esta técnica guiada por resonancia magnética de ultrasonidos de alta intensidad permite una ablación térmica no invasiva en el paciente del núcleo ventral intermedio del tálamo (VIM) con el fin de erradicar los temblores. El temblor esencial se caracteriza por un temblor general como único síntoma, sin otras características neurológicas que hagan pensar que podría ser otro tipo de patología. Muchos pacientes optan por tratamiento farmacológico, sin embargo, pueden llegar a generar resistencia, y en muchos de los casos no proporciona una mejoría de calidad de vida que sea satisfactoria. El objetivo principal ha sido observar tendencias de las variables preoperatorias, cuantificación de las escalas de temblor y tamaños de lesión durante la progresión temporal desde el procedimiento quirúrgico. También se han empleado correlaciones con el fin de observar si existe alguna relación entre las variables anteriormente mencionadas y los efectos secundarios. Se obtiene una diferencia significativa entre las escalas basales y aquellas tomadas al mes, así como con el control semestral. Entre la visita mensual y semestral se observa un pequeño repunte no significativo. En cuanto a las variables preoperatorias se concluye que aquellos pacientes con mayor SDR alcanzan con menores energías mayores temperaturas. Al analizar las lesiones se encuentra diferencia significativa de tamaño entre la lesión inmediatamente tras el procedimiento y la lesión mensual, siendo esta mucho mayor debido al edema vasogénico y citotóxico, pero no hay diferencia significativa entre la lesión inicial y la semestral. Además, aquellos outliers obtenidos en segmentación, explicarán algunos de los outliers obtenidos en la puntuación de la gravedad de los efectos secundarios. Los resultados obtenidos son de gran relevancia, pues con una base de datos relativamente pequeña se han observado tendencias que podrían apuntar a un comportamiento esperado de cada paciente, si se lograra aumentar la base de pacientes se podrían consolidar o rechazar por completo estas relaciones encontradas y ayudar a comprender esta nueva terapia en mayor profundidad.

[EN] The proposed final degree project is based on the analysis of the MRgFUS treatment of patients with essential tremor. This high-intensity ultrasound MRI-guided technique enables non-invasive thermal ablation of the patient's ventral intermediate nucleus of the thalamus (VIM) in order to eradicate tremors. Essential tremor is characterized by a general tremor as the only symptom, without other neurological characteristics that suggest that it could be another type of pathology. Many patients opt for pharmacological treatment, however, they can generate resistance, and in many cases it does not provide a satisfactory improvement in quality of life. The main objective was to observe trends in preoperative variables, quantification of tremor scales and lesion sizes during time progression from the surgical procedure. Correlations have also been used in order to see if there is any relationship between the aforementioned variables and secondary effects. A significant difference is obtained between the baseline scales and those taken a month, as well as with the semi-annual control. Between the monthly and semi-annual visits, a small non-significant rebound is observed. Regarding the preoperative variables, it is concluded that those patients with higher RDS reach higher temperatures with lower energies. When analyzing the lesions, a significant difference in size was found between the lesion immediately after the procedure and the monthly lesion, the latter being much larger due to the vasogenic and cytotoxic edema, but there was no significant difference between the initial lesion and the six-monthly lesion. In addition, those outliers obtained in segmentation will explain some of the outliers obtained in the side effect severity score. The results obtained are highly relevant, since with a relatively small database, trends have been observed that could point to an expected behavior of each patient. If the patient base were increased, these relationships could be consolidated or completely rejected. help understand this new therapy in greater depth.
