原发性高血压患者血压变异性与血浆神经肽Y 及神经降压素水平关系研究. (Chinese)

Objective To investigate the rationship between blood pressure variability and the plasma neuropeptide Y (NPY)and neurotensin(NT)in patients with essential hypertension.Methods 60 patients with essential hypertension admit⁃ ted to the General Hospitsal of Chongqing from December 2014 to June 2017 were divided into three groups according to the level of blood pressure:essential hypertension grade 1(group 1,n=22),essential hypertension grade 2(group 2,n=20)and essential hypertension grade 3(group 3,n=18),meanwhile,20 healthy subjects were selected control group.24 hours ambulatory blood pressure monitoring and detection of plasma NPY and NT were performed in each group.Results 24 hour systolic blood pressure variability(24hSBPV),daytime systolic blood pressure variability(DSBPV),night systolic blood pressure variation (NSBPV),24 hour diastolic blood pressure variability(24hDBPV),daytime diastolic blood pressure variability(DDBPV), night diastolic blood pressure variation(NDBPV)of patients with hypertension(including group 1,group2,group3)were signifi⁃ cantly higher than those of control group,the above indicators of group3 were significantly higher than those of group2 and group 1,the above indicators of group2 were significantly higher than those of group 1,the differences were statistically signifi⁃ cant(P<0.05).The level of plasma NPY of patients with hypertension(including group 1,group2,group3)were significantly higher than those of control group,group3 was significantly higher than group2 and group 1,group2 was significantly higher than group 1,the differences were statistically significant(P<0.05);The level of plasma NT of patients with hypertension(includ⁃ ing group 1,group2,group3)were significantly lower than those of control group,group3 was significantly lower than group2 and group 1,group2 was significantly lower than group 1,the differences were statistically significant(P<0.05)。Pearson correlation analysis showed that 24hSBPV,DSBPV,NSBPV,24hDBPV,DDBPV,NDBPV and NPY had a positive correlation(P<0.05), and NT had a negative correlation(P<0.05).Conclusion There is a positive correlation between blood pressure variability and NPY,and negatively correlated with NT,NPY and NT may be involved in the pathogenesis of blood pressure variability. 目的探讨原发性高血压患者血压变异性(BPV)与血浆神经肽Y(NPY)及神经降压素(NT)水平关系。 方法选择2014 年12 月至2016 年6 月重钢总医院收治的原发性高血压患者60 例,根据血压水平分为原发性高血压 1 级(高血压1 组,22 例)、原发性高血压2 级(高血压2 组,20 例)及原发性高血压3 级(高血压3 组,18 例),同时选择 健康体检者20 例作为对照组。各组均进行24 h 动态血压监测,同时检测血浆NPY 及NT 水平。结果高血压患者(高 血压1、2、3 组)的24 小时收缩压变异(24hSBPV)、昼间收缩压变异(DSBPV)、夜间收缩压变异(NSBPV)、24 小时舒张 压变异(24hDBPV)、昼间舒张压变异(DDBPV)、夜间舒张压变异(NDBPV)均高于对照组,高血压3 组以上指标均显著 高于高血压1、2 组,高血压2 组以上指标显著高于高血压1 组,差异均有统计学意义(P<0.05);高血压患者(高血压1、 2、3 组)的血浆NPY 水平均高于对照组,高血压3 组均显著高于高血压1、2 组,高血压2 组显著高于1 组,差异均有统 计学意义(P<0.05);高血压患者(高血压1、2、3 组)的血浆NT 水平均低于对照组,高血压3 组均显著低于高血压1、2 组,高血压2 组显著低于高血压1 组,差异均有统计学意义(P<0.05)。Pearson 相关性分析显示,24hSBPV、DSBPV、NSBPV、 24hDBPV、DDBPV、NDBPV 均与NPY 呈正相关(P<0.05),与NT 呈负相关(P<0.05)。结论血压变异性升高与NPY 存 呈正相关,与NT 呈负相关,NPY 及NT 可能参与了血压变异发病机制。 [ABSTRACT FROM AUTHOR]
