高血压合并心力衰竭患者脑钠肽与降钙素原相关性分析. (Chinese)

Objective To explore the correlation between brain natriuretic peptide (BNP) and procalcitonin (PCT) in the inpatients with essential hypertension complicating cardiac failure and its influence on the hypertension classification and cardiac function classification. Methods 160 inpatients with essential hypertension in Wenjiang District Hospital of Traditional Chinese Medicine from January 2012 to December 2014 were retrospectively analyzed, among them, 120 cases of complicating cardiac failure were taken as the observation group, including 38 cases of hypertension grade 1,46 cases of hypertension grade 2,36 cases of hypertension grade 3 and 3 cases of hypertension grade 4;58 cases of cardiac function grade II, 53 cases of cardiac function grade III and 11 cases of cardiac function grade IV according to the New York Heart Association standard (NYHA). Contemporaneous 40 patients with essential hypertension were selected as the control group. The differences in the BNP and PCT levels and their correlation were analyzed and compared. The relation between BNP and PCT with the hypertension classification and cardiac function classification in the patients with essential hypertension complicating cardiac failure was analyzed. Results BNP was positively correlated with PCT in the patients with essential hypertension complicating cardiac failure (r=0.706, P<0.01), the BNP and PCT levels in the observation group were significantly increased compared with the control group, the differences were statistically significant (P<0.05). The plasma BNP and PCT levels in the observation group had statistical differences among different hypertension grades and cardiac function grades, the differences were statistically significant (P<0.01). Conclusion Plasma BNP and PCT can serve as the monitoring indicators for the cardiac function insufficiency in patients with essential hypertension complicating cardiac failure. 目的 探讨原发性高血压合并心力衰竭住院患者血浆脑钠肽(BNP)、降钙素原(PCT)的相关性及对高 血压分级、心功能分级的影响。方法 回顾性分析温江区中医院2012年1月至2014年12月住院治疗的原发性高血压 患者160例, 其中将合并心力衰竭的120例患者设为观察组, 高血压分级1级38例, 2级46例,3级36例;纽约心脏病 协会(8YHA)心功能分级II级58例, III 级51例, IV 级11例。40例单纯原发性高血压患者为对照组。分析比较两组BNP、 PCT水平差异及二者相关性, 分析原发性高血压合并心力衰竭患者BNP、PCT与高血压分级和心功能分级的关系。结果 观察组患者88>与>?@呈正相关(r=0.706, P<0.01), 观察组BNP、PCT水平较对照组明显升高, 差异均有统计学意义(P< 0.05)。观察组患者中不同高血压分级和心功能分级患者间血浆BNP、PCT水平比较, 差异均有统计学意义(P<0.01)。结 论血浆BNP与PCT可作为高血压合并心力衰竭患者心功能不全的监测指标。 [ABSTRACT FROM AUTHOR]
