Objective To investigate the relationship between the different ambulatory arterial stiffness index and the markers of renal impairment in order to provide a scientific method for detecting the renal impairment of essential hypertension. Methods Three hundred essential hypertensive patients without overt proteinuria were enrolled. The ABPM was performed and the blood pressure parameters were analyzed in order to estimate the symmetrical ambulatory arterial stiffness index (S-AASI) and ambulatory arterial stiffness index (AASI) . Microproteinuria was measured by urine microalbumin to creatinine ( mAlb/Cr) as well as n-acetyl-β-d-glucosaminidase (NAG) to creatinine rate (NAG/Cr) . Creatinine clearance (Ccr) and Glomerular filtration rate (eGFR) were estimated from serumcreatinine (sCr) . Linear correlations were performed to confirm the independent predictive power of S-AASI and AASI for renal lesion. Results Correlation test showed a significant positively relationship of S-AASI with urine mAlb/Cr (0.708, P<0.001), urine NAG/Cr (0.700, P < 0.001) and sCr (0.229, P <0.05) . Ccr (0.601, P<0.001) and eGFR (0.309, P<0.05) were negatively correlated with S-AASI. On the other hand, AASI was also correlated with urine mAlb/Cr (0.489, P<0.001), urine NAG/Cr (0.470, P<0.001) and Ccr (0.311, P<0.05), but not with the sCr (0.064, P >0.05) and eGFR (-0.135, P >0.05) . S-AASI seems to get an independent relationship with all of the parameters of renal impairment which could not be detected with AASI. Conclusion This results suggested that S-AASI may be a better approach than AASI to estimate hypertensive renal impairment. [ABSTRACT FROM AUTHOR]
目的探讨两种动态动脉硬化指数与早期肾损害征象间的相关性.方法选取原发性高血压患者 300 例, 利用动态血压参数计算对称动态动脉硬化指数 (S-AASI) 、动态动脉硬化指数 (AASI), 检测尿微量白蛋 白 (mA1b) 、尿N- 乙酰-β-D- 氨基葡萄糖苷酶 (NAG), 计算肌酐清除率 (Ccr) 和评估肾小球滤过率 (eGFR) .行S-AASI、AASI 与早期肾损害指标间的Pearson 相关回归分析.结果S-AASI 与尿mA1b (0.708, P <0.01) 、NAG (0.700, P<0.01) 、sCr (0.229, P<0.05) 呈正相关, 与eGFR (-0.309, P<0.05) 、Ccr (-0. 601, P<0.01) 呈负相关.AASI 与尿mA1b (0.489, P<0.01) 、NAG (0.470, P<0.01) 、Ccr (-0.311, P<0.05) 相关性也有统计学意义.但与sCr (0.064, P>0.05) 和eGFR (-0.135, P>0.05) 不相关.结论两种动态动脉 硬化指数与早期肾脏损伤相关, S-AASI可能是比AASI 更灵敏的一种检测指标 [ABSTRACT FROM AUTHOR]
