The prevalence of primary arterial hypertension in children is increasing. Although hypertension in children is mostly asymptomatic, subclinical changes on target organs are already recorded. The aim was to analyze risk factors for arterial hypertension and clinical profiles in adolescents with primary hypertension. Retrospective analysis was performed on 153 children aged 10-18 years (117 M and 36 F) with primary hypertension, hospitalized during a 10-year period. Patients were divided into three groups: prehypertension, stage 1 hypertension and stage 2 hypertension. On statistical analysis, Student's t-test was used. The level of significance was set at p<0.05. Stage 1 hypertension was found in 27.5%, stage 2 hypertension in 65.4% and prehypertension in 7.1% of study patients. The majority of children (89.8%) had a positive family history of hypertension. Body mass index above the 95th percentile was recorded in 41.2% of patients, above 30 kg/m2 in 18.9% of them; children with stage 2 hypertension were more obese than children with stage 1 hypertension (p=0.005). In both hypertensive groups, the unfavorable form of non decreasing pressure (non dipper) was more frequent. Left ventricular hypertrophy was present in 15% of patients. Laboratory results (blood glucose, urate, total cholesterol and triglycerides) did not diff er statistically significantly between the two hypertensive groups. Hepatic steatosis was found in 5.9% and hypertensive retinal abnormalities in 1.6% of patients, both in the stage 2 hypertension group. Antihypertensive therapy, mostly angiotensin-converting enzyme inhibitors, was initiated in 20.3% of patients. In conclusion, a significant number of adolescents with primary hypertension are adipose and have a positive family history of hypertension. Prevention of hypertension should begin in childhood. Prevalencija primarne arterijske hipertenzije (AH) u djece je u porastu. Iako je hipertenzija u pedijatrijskoj dobi većinom asimptomatska, već se registriraju subkliničke promjene ciljnih organa. Cilj istraživanja je bio analizirati čimbenike rizika za AH i kliničke značajke adolescenata s primarnom hipertenzijom. Retrospektivno su analizirani podatci 153-oje bolesnika u dobi od 10 do 18 godina (117 M, 36 Ž) s dijagnozom primarne AH-a, hospitaliziranih u tercijarnoj zdravstvenoj ustanovi tijekom desetogodišnjeg razdoblja. Razdijeljeni su u tri skupine - djeca s prehipertenzijom, hipertenzijom I. stupnja (HT I.) i hipertenzijom II. stupnja (HT II.). Za statističku analizu primijenjen je Studentov t-test, razina značajnosti p<0,05. HT I. imalo je 27,5%, HT II. 65,4%, prehipertenziju 7,1% djece. Pozitivnu obiteljsku anamnezu za hipertenziju imalo je 89,8%, športski aktivnih bilo je 37,3%. Indeks tjelesne mase (ITM) iznad 95. centile imalo je 41,2%, iznad 30 kg/m2 18,9% ispitanika. Djeca s HT-om II. bila su deblja nego djeca s HT-om I. (p=0,005). U obje hipertenzivne skupine prevladava nepovoljniji, neopadajući obrazac tlaka "non dipper". Hipertrofi ja lijeve klijetke bila je prisutna u 15% djece. Usporedbom laboratorijskih vrijednosti GUK-a, urata, ukupnog kolesterola i triglicerida između skupina s HT-om I. i HT-om II. nije nađena statistički značajna razlika. Steatozu jetre imalo je 5,9%, hipertenzivne promjene očne pozadine 1,6% ispitanika, oboje u skupini s HT-om II. Antihipertenzivna terapija je uvedena kod 20,3% pacijenata, većinom inhibitori angiotenzin-konvertirajućeg enzima. Značajan broj naših adolescenata s primarnom hipertenzijom je adipozan i ima pozitivnu obiteljsku anamnezu za hipertenziju. Prevencija hipertenzije trebala bi početi već u djetinjstvu.
