Correlation between Plasma Homocysteine Levels and Brachial Ankle Pulse Wave Velocity, Carotid Atherosclerosis in Essential Hypertension Patients.

Objective To explore the impact of plasma homocysteine (Hcy) levels on the vascular function and the structure of essential hypertension (EH) patients. Methods Eighty-six consecutive outpatients with essential hypertension without drug treatment during September 2011 to February 2012 in our hospital were allocated into H-type hypertension group and a no-H-type hypertension group according to the plasma Hcy levels. There were 57 patients in H-type hypertension group and 29 in no-H-type hypertension group. At the same time, the baPWV was recorded, the carotid intima-media thickness (cIMT) was measured by ultrasonic testing and the carotid atherosclerosis plaque was screened. Results (1) In H-type hypertension group, the age of patients was older, the proportion of males, plasma levels of Hcy and creatinine (Cr), the detection rate of abnormal baPWV, cIMT, the detection rate of carotid plaque, and 24 hour mean systolic blood pressure standard deviation were higher than those in no-H-type hypertension group (P<0.05) . (2) After adjusting the factors of age, sex, lipid spectrum, glucose and ambulatory blood pressure, the plasma levels of Hcy and baPWV were still positively correlated (r=0.331, P = 0.007), the plasma levels of Hcy were still positively correlated with cIMT (r = 0.250, P = 0.037) by partial correlation analysis. Conclusions The plasma level of Hcy is positively correlated with baPWV and cIMT in patients with essential hypertension, and it can be used as an independent predictor. This study suggested that the elevated Hcy levels may be involved in pathological process of atherosclerosis in patients with essential hypertension. [ABSTRACT FROM AUTHOR]
